I hereby agree with the following:

I am participating in sessions during which I will receive information and instruction about meditation and meditative Inquiry. I represent and warrant that I have no mental health issues that would prevent my safe participation in these sessions.
Should any physical or mental health issues arise I agree to make them known.
In consideration of being permitted, I agree to assume full responsibility for any risks, injuries or damage, known and unknown, which I might incur as a result of participating.
In further consideration of being accepted, I knowingly, voluntarily, and expressly waive any claim I may have against the PRACTITIONER Melanie Schwindt, Untapped Resources, or if sessions are held in person to the owner/leaseholds of the building in which sessions are held.

I have read the above release and waiver of liability and fully understand its contents.
